CaseTrac
In consultation with Carmarthenshire Local Health Board, JML has successfully developed and deployed a complete ‘one stop shop’ web-application for managing their Condition Management Programme (CMP).

CaseTrac is deployed on an intranet, allowing easy access by authorised staff from any computer on the intranet or with the right set up from external computers for home workers. CaseTrac monitors all cases, from referral through to discharge, with special management reports including:

• Deadline Manager – for monitoring timely progression of cases.
• Deadline Performance – for measuring progression of cases against targets.
• Case Analysis – analysis of cases and conditions for MI returns.
• Discharge Analysis – analysis of discharge reasons for MI returns.
• Event Schedule– calendar of interventions that have been arranged for clients.

In addition, CaseTrac has comprehensive searching and breakdown features e.g.:

• Break down all cases by Gender to find the number of males versus female.
• Then break down the male cases by Age to see a distribution across the age ranges.
• Then break down the male over-50s by Condition to find out which health conditions they are being referred with, and so on.

CaseTrac stores Clients, Practioners, Cases, Interventions/Appointments, GPs, GP Practices, Job Centres, Areas etc., all of which can be tailored by the customer to their local situation. The system is underpinned by a complete audit trail that captures every change made to the data, who made it and when.

As CaseTrac is a web-application, it would be possible to add a web-services interface that could be queried by a central master application. Data could then be collated automatically from CMP providers all over the country. This would completely remove the time and expense of having to manually report statistics to wider organisations, while giving those at the centre a real-time national perspective on CMP delivery.
